Medically Necessary Eye Exams

“Eyes Forward” is a program in which we partner with local optometry practices to increase access to medically necessary eye exams. If you have diabetes, hypertension or another acute illness, an annual dilated exam is necessary to protect and preserve the health of your eyes. Eyes Forward can assist in paying for your medically necessary eye exam if you qualify. For more information eligibility requirements, please contact our health centers.
