Whether you are a new patient looking for a primary care provider, dental care, preventive services, or ongoing healthcare support, we are here to assist you in improving and maintaining your overall health and wellness.

Connect Health + Wellness is proud to serve as a health center that provides accessible and affordable healthcare for our community. As a Federally Qualified Health Center (FQHC), we welcome patients of all backgrounds, including those who are uninsured or underinsured. For new patients, our new patient packet includes all the essential information you need to get started with our services. We accept most insurance plans, including Medicare and Medicaid, and for those who qualify, our Sliding Fee Discount Program may help reduce the cost of patient care.

Getting started at our health center is easy: 

Complete a new patient registration packet online or visit any of our health centers to pick one up. Once you've filled out your forms, return them to your preferred Connect Health + Wellness location. 

You can easily schedule your first appointment with our care team online or by contacting one of our health centers for assistance. New patients who book their appointments online can complete any required paperwork during their visit.

Connect Health + Wellness provides quality healthcare services for patients of all ages, including:
