The RN Case Manager plays a critical role in coordinating, managing, and delivering high quality care for patients across multiple clinic sites. This position focuses on preventive care, chronic disease management, and transitional care services while ensuring patients receive comprehensive, individualized care. The RN Case Manager will perform annual wellness visits, chronic care management, and care coordination activities, including care plan development and follow-up. Travel between clinics and sites is a regular expectation of this role.


Key Responsibilities of the position

Annual Wellness Visits (AWVs)

  1. Conduct AWVs in accordance with Medicare guidelines.
  2. Assess patient health status, review medications, and identify preventive care needs.
  3. Document and communicate findings to the care team.

Chronic Care and Case Management

  1. Provide structured care management for patients with chronic conditions.
  2. Monitor patient progress and adherence to care plans.
  3. Educate patients and families on disease management, lifestyle modifications and self-care strategies.
  4. Coordinate care across multiple providers and clinic sites.
  5. Identify high-risk patients and develop individualized care plans.

Care Plan Development and Maintenance

  1. Develop, implement, and update comprehensive care plans tailored to each patient’s needs and in accordance with PCMH (Patient Centered Medical Home) guidelines.
  2. Ensure documentation meets regulatory, payer, and organizational standards.

Transition Care Management (TCM)

  1. Conduct post-discharge follow-ups with patients to ensure continuity of care.
  2. Coordinate with hospital and specialty providers to prevent readmissions.

Patient Education and Advocacy

  1. Provide counseling and education to patients and families regarding health conditions and treatments.
  2. Advocate patient needs within the healthcare team and the broader community.

 Connect Health + Wellness – Bassett is seeking candidates for Referral Coordinator at our Bassett Virginia location.

This is a customer service position. The Referral Coordinator is in frequent contact with Health Center patients, their families and a variety of third-party vendors and stakeholders. As such, it is important that this person has a very pleasant disposition and can communicate well with a wide range of people.


The candidate will perform various front office administrative activities for a fast-paced Health Center to ensure smooth operations, excellent patient relations and the highest levels of service quality and professionalism. Candidate interacts with patients, patients’ families, medical providers, and others regarding patient-related matters. This position requires someone who can work independently and carry out assignments with minimal instruction. This position demands adherence to prescribed routines, maintaining records and data entry with a high level of accuracy and speed. This person must be able to work in a high stress environment and must be bondable. This person will work with other clinic staff in a Patient Centered Medical Home environment that values teamwork and relentlessly pursues the highest levels of quality care for our patients.


Essential Functions of the Position

  1. Manages patient referrals and Diagnostic Imaging orders. Identifies patient Provider needs and preferences, verifies medical record information and then assists patients by contacting specialists and other third-party facilities and making appointments on the patient’s behalf. Communicates appointments (date, time, location, Provider) to the patients and records on the Electronic Medical Record (EMR). Specialty services include, but are not limited to, x-rays, colonoscopy consults, mammograms, MRI’s CT’s, echocardiograms, nuclear medicine, and sleep studies. Identifies, secures, and manages free services from Provider specialists. Secures pre-authorizations as needed. Tracks and records relevant data in spreadsheets and records related activities in the EMR.
  2. Educates uninsured and/or underinsured patients about available financial assistance programs and assists in preparing and submitting corresponding applications. Then verifies receipt and outcome and records in the EMR (Electronic Medical Record).
  3. Eyes Forward appointments: Tracks and records relevant payment and demographic information and provides this information to the Patient Care Coordinator and Billing Specialist as required.
  4. Pregnancy referrals for UDS measurements; Tracks and records relevant referral appointment, demographic and outcome data.
  5. Assists patients in completing medical record requests. Assembles necessary medical records (labs, DI, notes, etc.) and forwards them to the specialist or other third-party Providers.
  6. Tracks all referrals on a referral log/spreadsheet.
  7. Troubleshoots referrals that have experienced service or other logistical difficulties.
  8. Input and update patient demographic information in the EMR with a high degree of accuracy. Scan patient documents into the electronic medical record and forward them to the Provider(s) as needed.
  9. Provides back up/assistance with various other administrative tasks as needed.
  10. Leads the P2P (Provider to Provider) initiative for the practice to ensure the Meaningful Use measure is met.
  11. Develops and uses good organizational, analytical, numerical, and reasoning skills.
  12. Manages the electronic fax inbox, sorting and processing all received faxes in a timely manner.
  13. Participates in training(s) opportunities to improve patient care including, but not limited to, proactively addressing the needs of patients and families in this community, cultural competency, effective communication, OSHA, HIPAA and EHR.

Connect Health +Wellness is seeking candidates for a part-time (up to 29 hours per week) Care Management Coordinator for our organization, working out of our Primary Care clinic in Ridgeway, Virginia.

The Care Management Coordinator assists the Quality Department by working with the Connect Health + Wellness staff to support a patient-centered model of care. Under direct supervision, the Care Management Coordinator assists with identification of patients with complex issues and/or those needing additional screening, as well as providing additional assistance, outreach, and follow-up. This person will work with other staff in a Patient Centered Medical Home environment the values teamwork and relentlessly pursues the highest levels of quality care for our patients.


Essential Functions of the Position

  1. Assists with identification of patients who have complex medical issues (such as those having several diagnoses, chronic illnesses, complicated treatments, multiple providers) to aid in care coordination.
  2. Assists in managing patient caseload and identifying patients that require additional assistance with barriers to care.
  3. Ensures that patients have appropriate follow-up after ED/Hospital Discharge to decrease hospital re-admissions.
  4. Ensures that patients with missed appointments are rescheduled in a timely manner to better serve our patient population.
  5. Reaches out to patients with identified care gaps to meet required value-based measures.
  6. Participates in quality meetings to maintain patient-focused care and improved outcomes for patients.

Connect Health + Wellness is seeking candidates for an experienced Licensed Practical Nurse (LPN) at our Bassett, Virginia locations.


Connect Health + Wellness is committed to providing primary health, dental and integrated behavioral health services at our Federally Qualified Health Center sites and our Dental Clinics by promoting health, reducing health risk factors, and increasing access to medical and dental services, primarily for the uninsured and underserved in our service area. Additionally, Connect Health + Wellness offers chronic disease self-management, medication assistance, and care coordination services to patients in our service area.


Our LPN's provide clinical support in the delivery of quality care for the patient in a medical practice setting. Must have at least 1 year experience in healthcare, preferably in a primary care or urgent care setting working with a Provider. Candidates should be familiar with use of an electronic medical record (EMR), preferably eClinical Works (eCW), and knowledgeable about care coordination and health education.


Qualified candidates can provide professional nursing care according to current best practices, maintain confidentiality, promote person-centered care, communicate effectively and have good computer skills. Must be team oriented, a problem-solver and have excellent organizational and time management skills.


Job Duties

LPN job duties include, but may not be limited to the following:

  • Completes patient assessment to include height, weight, vital signs, chief complaint, current medications, etc.
  • Prepares patient for provider evaluation, including obtaining specimens, positioning for exams, listing medications, assisting during exams and diagnostic and therapeutic procedures.
  • Administers medications and injections as directed by the provider.
  • Maintains work area, exam, treatment rooms in a clean, safe manner; restock with supplies and instruments; sterilize reusable equipment as needed.
  • Documents patient information in the electronic medical recorded.
  • Provides patient teaching as indicated and directed by the provider including lab results, diet instructions, medication instructions, appropriate follow-up, and reinforcing provider instructions as needed.
  • Assist with referral authorizations for specialists and pre-authorizations for testing and ancillary services.
  • Assists with lab procedures.
  • Shift will vary based on the Provider's schedule.

The Family Medicine Physician delivers comprehensive, patient-centered primary care to individuals of all ages in a Federally Qualified Health Center (FQHC) setting. Working as part of an interdisciplinary care team, the physician plays a critical role in improving access to high-quality, affordable care with a focus on prevention, chronic disease management, and coordination of care that addresses medical, behavioral, and social determinants of health.

This role requires adherence to current clinical standards, FQHC regulatory guidelines, and organizational protocols. The physician is also expected to actively engage in quality improvement initiatives and collaborate with staff to support a culture of excellence, equity, and continuous learning.


Essential Duties and Responsibilities

  1. Provide evidence-based, comprehensive primary and preventive care to a diverse patient population.
  2. Order, interpret, and evaluate diagnostic tests to inform clinical decision-making.

The Certified Medical Assistant provides basic technical clinical services and clerical support in the delivery of quality patient care as directed by licensed staff and providers.

The CMA must be able to work effectively in a fast-paced environment, employing exceptional critical thinking and problem-solving skills while providing efficient and accurate healthcare. This person serves as a patient liaison and assists in providing a continuum of healthcare in keeping with the goals and objectives of a Patient Centered Medical Home.


The CMA is responsible for tracking and recording various quality assurance measures and outcomes and ensuring that Quality Assurance and Performance Improvement standards and requirements are met. This position assists with Health Coaching, especially regarding patients with the three chronic disease conditions and/or high-risk criteria identified by the Patient Centered Medical Home. This position demands adherence to prescribed routines, maintaining records, data, and other information with a high degree of accuracy and speed. This position is an integral member of a Patient Centered Medical Home team. This individual must be able to work in a fast-paced, stressful environment and relentlessly pursue the highest levels of quality care for our patients.


Job Duties - Clinical

  1. Participates in daily “huddles” with Providers to address all patient needs prior to each patient’s appointment to help ensure quality, evidence-based care including preventative screenings, chronic disease management and review of care plans.
  2. Escort patients to the exam area, providing assistance when needed. Ensure that patients and families understand the treatment care plan and educate them relative to their condition.
  3. Performs patient assessments including obtaining vital signs, gathering the patient’s social, medical, and surgical history, recording patient medications/history, copying immunizations records and other pertinent information related to the appointment. Records information and data in the Electronic Medical Record (EMR).
  4. Administers prescribed medications as directed by Providers (including orally, intramuscular, z-track, subcutaneous and intradermal).
  5. Uses sterile techniques when preparing and assisting with procedures and applying dressings.
  6. Collects, prepares, and labels laboratory specimens according to established policies and guidelines.
  7. Cleans and sterilizes instruments and equipment in accordance with established policies and guidelines (i.e., using the designated disinfecting solution and/or the autoclave).
  8. Monitors supplies and sterile instrument packages to ensure that they are not outdated or otherwise compromised. Keeps crash cart and refrigerator logs up to date.
  9. Re-stocks exam room supplies daily, and procedure room supplies as needed.
  10. Performs audiometer, vision, tympanometry and spirometer tests. Records results, and ensures that related documents are scanned, into the patient’s EMR.
  11. Performs quality assurance control checks for medical equipment and lab tests as directed.
  12. Accurately records patient information, lab results and related documents in the EMR.
  13. Assists Providers with management of an assigned segment of patients in order to provide awareness of chronic disease controls and assist them in obtaining desired clinical outcomes, including reviewing and documenting self-management goals. This may also include ensuring that patients are referred to, and attend, Connect Health + Wellness education classes.
  14. Identifies patients in assigned population who have outstanding lab work or clinical exams. Makes contact with these patients to ensure necessary appointments are made. Reviews the patient schedule (weekly) to confirm that appointments are kept. Contacts patients as necessary to determine reasons that appointments were not kept and works with patients to ensure appropriate care. Identifies and communicates noncompliant patients to the Provider (weekly).
  15. Participates in the Quality Assurance program. Identify and records barriers to care, particularly for their assigned segment of patients. Develops and communicates ideas for breaching or overcoming such barriers to the Quality Assurance/Performance Improvement Committee monthly.
  16. Ensures that all telephone messages and encounters are assigned/returned within twenty-four (24) business hours. This includes contacting patients to relay Provider messages and/or answer to patient questions. Documents all patient interaction and resolutions in the EMR.
  17. Contacts patients to discuss lab/diagnostic results and relay Provider treatment instructions.
  18. Contacts other healthcare facilities to obtain copies of results for tests that they performed.
  19. Reviews appropriate consent forms with patients, ensuring their understanding, answering their related questions, and securing their permission as evidenced by their signature(s).
  20. Assists patients in completing the Controlled Substances Agreement, as applicable, and ensuring that related pharmacy information is entered/updated in the EMR.
  21. Obtains pre-authorization for medication from insurance companies and documents resolutions, including medication changes, in the EMR.
  22. Contacts hospitals on behalf of patients requiring admission and communicates relevant patient history and information as needed and appropriate.
  23. Assists with, and oversees the tracking of, patient referrals and outcomes.
  24. Manages Health Coaching and related support of patients.


Job Duties - General

Eligibility Counselor duties can include, but may not be limited to the following:

  • Prepare for Daily Operations: Review upcoming appointments and patient accounts in advance of scheduled visits to identify financial counseling needs.
  • Maintain Accurate Patient Records: Enter and update patient demographic, insurance, and financial information in the Electronic Medical Record (EMR) system with a high degree of accuracy.
  • Verify Insurance Coverage: Confirm insurance eligibility at each visit, obtain authorizations as needed, and scan insurance cards into the EMR. Assist patients with updating their Primary Care Provider (PCP) on file with their insurance plan when necessary.
  • Process Sliding Fee Discount Program Applications: Evaluate new and renewal applications for the Sliding Fee Discount Program, including those submitted outside of regular hours. Confirm patient eligibility and update EMR to reflect approved discount levels. Notify patients of expiring or incomplete documentation and assist with timely resubmission.
  • Manage Proof of Income (POI) Documentation: Collect and scan acceptable POI documentation into the EMR. Communicate clearly with patients about applicable co-pays, sliding scale fees, or financial obligations.
  • Coordinate with Front Office Team: Print and review the daily schedule to identify patients needing financial counseling. Communicate with front desk staff to ensure appropriate patient routine and handoffs.
  • Conduct Patient Financial Consultations: Meet with patients upon arrival to review account status, collect co-pays or outstanding balances, and establish payment plans with required documentation. Educate patients on their financial responsibility and available assistance options.
  • Facilitate Financial Hardship Applications: Provide hardship applications to qualifying patients, help in completing them, and ensure timely submission and follow-up.

The Billing and Coding Specialist at Connect Health + Wellness is responsible for the accurate assignment of procedural codes, submission of clean claims, and resolution of billing discrepancies. This position ensures that all clinical services, including medical, behavioral health, dental, and ancillary services, are coded and billed in compliance with FQHC-specific requirements, including Medicare PPS, Medicaid Managed Care, commercial payer guidelines, and HRSA standards. Medical Coding Responsibilities

  • Assign accurate CPT, HCPCS, and ICD-10-CM codes based on provider documentation for all service lines (Medical, Dental, Behavioral Health, and Ancillary).
  • Ensure appropriate use of modifiers and encounter-based billing codes (e.g., T1015, G-codes) in accordance with FQHC billing rules.

Cross-Functional Coordination
