Connect Health +Wellness is seeking candidates for a part-time (up to 29 hours per week) Care Management Coordinator for our organization, working out of our Primary Care clinic in ridgeway, Virginia.


Connect Health +Wellness is committed to providing primary health, dental and integrated behavioral health services at our Federally Qualified Health Center sites and our Dental clinics by promoting health, reducing health risk factors, and increasing access to medical and dental services, primarily for the uninsured and underserved in our Service Area. Additionally, we offer chronic disease self-management, medication assistance, and care coordination services to patients and local residents.


The Care Management Coordinator assists the Quality Department by working with the Connect Health + Wellness staff to support a patient-centered model of care. Under direct supervision, the Care Management Coordinator assists with identification of patients with complex issues and/or those needing additional screening, as well as providing additional assistance, outreach, and follow-up. This person will work with other staff in a Patient Centered Medical Home environment the values teamwork and relentlessly pursues the highest levels of quality care for our patients.


Essential Functions of the Position

  1. Assists with identification of patients who have complex medical issues (such as those having several diagnoses, chronic illnesses, complicated treatments, multiple providers) to aid in care coordination.
  2. Assists in managing patient caseload and identifying patients that require additional assistance with barriers to care.
  3. Ensures that patients have appropriate follow-up after ED/Hospital Discharge to decrease hospital re-admissions.
  4. Ensures that patients with missed appointments are rescheduled in a timely manner to better serve our patient population.
  5. Reaches out to patients with identified care gaps to meet required value-based measures.
  6. Participates in quality meetings to maintain patient-focused care and improved outcomes for patients.
  7. Participates in training(s) opportunities to improve patient care including, but not limited to, proactively addressing the needs of patients and families in this community, cultural competency, effective communication, OSHA, HIPAA and EHR.
  8. Works under the guidance of  the Quality Coordinator.
  9. Assists with accurate data collection and reporting.
  10. Performs these and other duties and responsibilities as assigned by the Quality Coordinator.

No nights or weekend work


Minimum Required Education, Experience, Abilities & Characteristics

  • Minimum one year experience in a physician office preferred
  • Prior experience in data entry required
  • A diverse set of skills and abilities including organization, prioritization, good grammar, telephone and interpersonal business manners and the ability to multitask
  • Good computer skills including knowledge of Electronic Medical Record systems (EMRs)
    • Experience with eClinicalWorks (eCW) EMR, preferred but not required
  • A passion and concern for the target population served by the organization
  • Effective communication skills to work with the public
  • Licensure/Certification: Current Medical Assistant Certification through a recognized national professional association is preferred but not required.
  • AHA BLS-HCP Certification required.

Connect Health + Wellness is seeking candidates for a part-time Dental Assistant for our Dental Clinic in Martinsville, Bassett and Patrick Springs, Virginia. (up to 29 hours per week)


Connect Health + Wellness is committed to providing primary health and integrated behavioral health services at our Federally Qualified Health Centers and Dental sites by promoting health, reducing health risk factors and increasing access to medical and dental services, primarily for the uninsured and underserved within our service area. Additionally, we offer chronic disease self-management, medication assistance, and care coordination services.


The Dental Assistant performs various activities for a fast-paced Dental Clinic including assisting the dentist as well as dental and dental hygiene students in the direct provision of dental services to patients of the dental clinic. The Dental Assistant is responsible for sterilization, preparation and inventory control of dental instruments and supplies, and taking needed x-rays.


Essential Functions of the position

1. Assist's patients in resolving minor difficulties, answering their questions, and giving directions to patients as authorized by the dentist or dental hygienist.

2. Serves as dentist's or dental hygienist's chair-side assistant.

3. Prepares operatory for patient treatment as per Dental Clinic protocols and the dentist's or dental hygienist's directions.

4. Exposes and develops dental radiographs in accordance with state regulations and law as well as Dental Clinic directive and protocol.

5. Performs independent procedures as delegated and directed by the dentist in accordance with state regulation and law and Dental Clinic directive and protocol.

6. Maintains Dental Clinic equipment in accordance with manufacturer's directions and Dental Clinic policy and protocol.

7. Maintains all Dental Clinic areas in compliance with Dental Clinic directives and protocols as well as center policies and procedures relative to infection control, exposure control and safety issues.

8. Maintains adequate operatory supplies and compiles a list of individual item shortages for inventory control and ordering purposes.

9. Accesses patient charts through EMR, as required to enter needed information.

10. Responds to and places necessary telephone calls as need for the Dental Clinic.

11. Assists with various clinical and administrative functions of the center as appropriate and time permits.

12. Insures the sterility of all reusable dental instruments and equipment in accordance with Dental Clinic directive and protocol.

13. Insures the proper disposal of all contaminated or potentially contaminated materials in accordance with Dental Clinic directive, center policy as well as state and federal regulation(s).

14. Performs all functions in full compliance with the clinics and the Dental Clinic Exposure Control Plan including but not limited to the use of Personal Protective Equipment and Universal Precautions.

15. Participates in appropriate health promotion / disease prevention activities, both on-site and off-site as required which may also include our mobile dental clinic.

16. Travels when necessary to meet operational needs.

17. As directed by Dental Practice Manager performs other related and/or necessary tasks to achieve organizational and program goals and objectives.

18. Responsible for full personal compliance with all applicable federal, state, local and center rules, regulations, protocols, and procedures including but not limited to the participation of a Dental Assistant in the provision of clinical dental care, as well as those relating to, but not limited to personnel issues, workplace safety, public health, and confidentiality.

19. Other duties as may be assigned from time-to-time by your supervisor that are consistent with the Mission of the organization and intent of this position.


EDUCATION AND TRAINING

  • High school diploma or GED
  • Completion of accredited course in dental assisting, preferred but not required
  • Effective oral and written communication skills in English are required.
  • Bilingual (English/Spanish) preferred but not required


LICENSURE AND CREDENTIALS

  • Certified Dental Assistant, preferred but not required
  • X-Ray Certification, preferred but can be completed upon hire
  • CPR (BLS), can provide training upon hire


SKILLS AND ABILITIES

  • Requires a comprehensive knowledge of dental assisting techniques and procedures for all phases of general dentistry.
  • Knowledge of modern dental materials, their storage, handling, and applications is required.
  • The Dental Assistant must be cognizant of the expectations and concerns of the Dental Clinic’s patients and be able to respond in an empathetic and professional manner.
  • The ability to work with people from a wide diversity of social, ethnic, and economic backgrounds is necessary.
  • The Dental Assistant must be able to work with other health care professionals from a variety of disciplines to achieve desired results for the patient from a system of integrated primary health care.
  • Bi-lingual (English/Spanish) helpful but not required


TYPICAL PHYSICAL DEMANDS

Connect Health + Wellness is seeking candidates for an experienced Licensed Practical Nurse (LPN) at our Bassett, Virginia locations.


Connect Health + Wellness is committed to providing primary health, dental and integrated behavioral health services at our Federally Qualified Health Center sites and our Dental Clinics by promoting health, reducing health risk factors, and increasing access to medical and dental services, primarily for the uninsured and underserved in our service area. Additionally, Connect Health + Wellness offers chronic disease self-management, medication assistance, and care coordination services to patients in our service area.


Our LPN's provide clinical support in the delivery of quality care for the patient in a medical practice setting. Must have at least 1 year experience in healthcare, preferably in a primary care or urgent care setting working with a Provider. Candidates should be familiar with use of an electronic medical record (EMR), preferably eClinical Works (eCW), and knowledgeable about care coordination and health education.


Qualified candidates can provide professional nursing care according to current best practices, maintain confidentiality, promote person-centered care, communicate effectively and have good computer skills. Must be team oriented, a problem-solver and have excellent organizational and time management skills.


Job Duties

LPN job duties include, but may not be limited to the following:

  • Completes patient assessment to include height, weight, vital signs, chief complaint, current medications, etc.
  • Prepares patient for provider evaluation, including obtaining specimens, positioning for exams, listing medications, assisting during exams and diagnostic and therapeutic procedures.
  • Administers medications and injections as directed by the provider.
  • Maintains work area, exam, treatment rooms in a clean, safe manner; restock with supplies and instruments; sterilize reusable equipment as needed.
  • Documents patient information in the electronic medical recorded.
  • Provides patient teaching as indicated and directed by the provider including lab results, diet instructions, medication instructions, appropriate follow-up, and reinforcing provider instructions as needed.
  • Assist with referral authorizations for specialists and pre-authorizations for testing and ancillary services.
  • Assists with lab procedures.
  • Shift will vary based on the Provider's schedule.

The Family Medicine Physician delivers comprehensive, patient-centered primary care to individuals of all ages in a Federally Qualified Health Center (FQHC) setting. Working as part of an interdisciplinary care team, the physician plays a critical role in improving access to high-quality, affordable care with a focus on prevention, chronic disease management, and coordination of care that addresses medical, behavioral, and social determinants of health.

This role requires adherence to current clinical standards, FQHC regulatory guidelines, and organizational protocols. The physician is also expected to actively engage in quality improvement initiatives and collaborate with staff to support a culture of excellence, equity, and continuous learning.


Essential Duties and Responsibilities

  1. Provide evidence-based, comprehensive primary and preventive care to a diverse patient population.
  2. Order, interpret, and evaluate diagnostic tests to inform clinical decision-making.
  3. Collaborate with internal staff and external consultants to ensure coordinated care.
  4. Maintain complete, accurate documentation in the Electronic Medical Record (EMR) system.
  5. Participate in quality assurance and policy development initiatives.

  13. Actively participate in peer review and continuous improvement efforts.
  14. Respond to phone calls and EMR communications within 48 hours.
  15. Meet or exceed benchmarks in Clinical Performance Measures (e.g., diabetes, hypertension, cancer screening).
  16. Contribute to patient outreach and growth efforts.

Our CMAs (CMA, CCMA or RMA) are routinely in direct contact with Health Center patients, their families and a variety of third-party vendors and stakeholders. As such, it is important that this person has a very pleasant disposition and can communicate well with a wide range of people.


The Certified Medical Assistant provides basic technical clinical services and clerical support in the delivery of quality patient care as directed by licensed staff and providers.

The CMA must be able to work effectively in a fast-paced environment, employing exceptional critical thinking and problem-solving skills while providing efficient and accurate healthcare. This person serves as a patient liaison and assists in providing a continuum of healthcare in keeping with the goals and objectives of a Patient Centered Medical Home.


The CMA is responsible for tracking and recording various quality assurance measures and outcomes and ensuring that Quality Assurance and Performance Improvement standards and requirements are met. This position assists with Health Coaching, especially regarding patients with the three chronic disease conditions and/or high-risk criteria identified by the Patient Centered Medical Home. This position demands adherence to prescribed routines, maintaining records, data, and other information with a high degree of accuracy and speed. This position is an integral member of a Patient Centered Medical Home team. This individual must be able to work in a fast-paced, stressful environment and relentlessly pursue the highest levels of quality care for our patients.


Job Duties - Clinical

  1. Participates in daily “huddles” with Providers to address all patient needs prior to each patient’s appointment to help ensure quality, evidence-based care including preventative screenings, chronic disease management and review of care plans.
  2. Escort patients to the exam area, providing assistance when needed. Ensure that patients and families understand the treatment care plan and educate them relative to their condition.
  3. Performs patient assessments including obtaining vital signs, gathering the patient’s social, medical, and surgical history, recording patient medications/history, copying immunizations records and other pertinent information related to the appointment. Records information and data in the Electronic Medical Record (EMR).
  4. Administers prescribed medications as directed by Providers (including orally, intramuscular, z-track, subcutaneous and intradermal).
  5. Uses sterile techniques when preparing and assisting with procedures and applying dressings.
  6. Collects, prepares, and labels laboratory specimens according to established policies and guidelines.
  7. Cleans and sterilizes instruments and equipment in accordance with established policies and guidelines (i.e., using the designated disinfecting solution and/or the autoclave).
  8. Monitors supplies and sterile instrument packages to ensure that they are not outdated or otherwise compromised. Keeps crash cart and refrigerator logs up to date.
  9. Re-stocks exam room supplies daily, and procedure room supplies as needed.
  10. Performs audiometer, vision, tympanometry and spirometer tests. Records results, and ensures that related documents are scanned, into the patient’s EMR.
  11. Performs quality assurance control checks for medical equipment and lab tests as directed.
  12. Accurately records patient information, lab results and related documents in the EMR.
  13. Assists Providers with management of an assigned segment of patients in order to provide awareness of chronic disease controls and assist them in obtaining desired clinical outcomes, including reviewing and documenting self-management goals. This may also include ensuring that patients are referred to, and attend, Connect Health + Wellness education classes.
  14. Identifies patients in assigned population who have outstanding lab work or clinical exams. Makes contact with these patients to ensure necessary appointments are made. Reviews the patient schedule (weekly) to confirm that appointments are kept. Contacts patients as necessary to determine reasons that appointments were not kept and works with patients to ensure appropriate care. Identifies and communicates noncompliant patients to the Provider (weekly).
  15. Participates in the Quality Assurance program. Identify and records barriers to care, particularly for their assigned segment of patients. Develops and communicates ideas for breaching or overcoming such barriers to the Quality Assurance/Performance Improvement Committee monthly.
  16. Ensures that all telephone messages and encounters are assigned/returned within twenty-four (24) business hours. This includes contacting patients to relay Provider messages and/or answer to patient questions. Documents all patient interaction and resolutions in the EMR.
  17. Contacts patients to discuss lab/diagnostic results and relay Provider treatment instructions.
  18. Contacts other healthcare facilities to obtain copies of results for tests that they performed.
  19. Reviews appropriate consent forms with patients, ensuring their understanding, answering their related questions, and securing their permission as evidenced by their signature(s).
  20. Assists patients in completing the Controlled Substances Agreement, as applicable, and ensuring that related pharmacy information is entered/updated in the EMR.
  21. Obtains pre-authorization for medication from insurance companies and documents resolutions, including medication changes, in the EMR.
  22. Contacts hospitals on behalf of patients requiring admission and communicates relevant patient history and information as needed and appropriate.
  23. Assists with, and oversees the tracking of, patient referrals and outcomes.
  24. Manages Health Coaching and related support of patients.


Job Duties - General

  1. Takes the initiative, follows directions, multi-tasks effectively and efficiently and communicates well with others.

Connect Health + Wellness is seeking a compassionate and skilled Dentist to join our team in providing high-quality clinical dental care to a diverse patient population at our Federally Qualified Health Center (FQHC). The ideal candidate will have a commitment to serving underserved communities and be ready to work alongside dental students, staff, and other healthcare providers to ensure comprehensive, patient-centered care.


Key Responsibilities:

  • Clinical Care: Provide a wide range of dental services, including examinations,      diagnostics, and treatments.
  • Supervision: Serve as an Adjunct Faculty Member for Virginia Commonwealth University School of Dentistry, supervising dental students in clinical settings.
  • Patient Education: Educate patients on oral health, preventive care, and available      treatment options.
  • Collaboration: Work closely with the CEO, Medical Director, and interdisciplinary team to improve service delivery and patient care.
  • Policy Development: Assist in developing and revising dental department policies and protocols to ensure the best practices.
  • Community Outreach: Engage in community outreach to educate and provide technical assistance related to oral health.
  • Program Planning: Participate in planning initiatives for the Dental Clinic and      broader organizational goals.
  • Compliance & Quality Assurance: Ensure adherence to regulations and contribute to quality assurance efforts.
  • Consultation: Provide expert consultation on dental health issues to the CEO, Board of Directors, and medical teams.

Required Qualifications:

  • Education: Graduation from an accredited dental school.
  • Experience: Previous experience in a clinical dental setting is preferred.
  • Licensure: Current, unrestricted license to practice dentistry in Virginia.
  • Certifications: AHA BLS-HCP certification required; ACLS certification preferred.
  • Skills: Strong communication, interpersonal, and organizational skills. Ability to      work effectively with diverse populations and as part of a team.
  • Additional Qualifications: 
    • Bilingual in English and Spanish is preferred.
    • Knowledge of public health principles is a plus.
    • Proficiency with dental software and Electronic Medical Records (EMR).
    • Experience working with underserved communities is highly valued.

Dental Front-Office Clerk (Part-time) could be up to 29 hours per-week.


This is a customer service position. The Dental Front Office Clerk is frequently the initial point of contact for patients and the last person to interact with our patients as they leave the Dental Clinic. Therefore, it is important that this person has a very pleasant disposition and can communicate well with a wide range of people.


This person performs various front office administrative activities for a fast-paced Dental Clinic Center to ensure smooth operations, excellent patient relations and the highest levels of service quality and professionalism. He or she interacts with patients, patients’ families, medical providers and others regarding patient-related matters.


This position requires a person who can work independently and carry out assignments with minimal instruction. ESSENTIAL FUNCTIONS OF THE POSITION

  3. Inputs and updates patient demographic information in an Electronic Medical Record (EMR) with a high degree of accuracy.

    • Keep current on payer updates, code set changes, and compliance policies through ongoing education.

Billing Responsibilities
Prepare, review, and submit timely and accurate claims to Medicare, Medicaid, MCOs, and commercial payers.

  • Ensure accurate application of the Sliding Fee Discount Schedule based on patient eligibility and income documentation. - Track and resolve denied, rejected, or underpaid claims by identifying root causes and correcting issues.
  • Post insurance payments, contractual allowances, and patient payments accurately into the billing system.
  • Monitor and follow up on outstanding accounts receivable to ensure timely reimbursement and minimize aging.

Cross-Functional Coordination

  • Collaborate with front desk staff and financial counselors to ensure accurate demographic, insurance, and eligibility data is captured at registration.
  • Assist with training staff on documentation requirements that affect billing and coding compliance.
  • Support Uniform Data System (UDS) and cost reporting by ensuring all billable encounters are accurately coded and billed.

Compliance and Documentation

  • Maintain strict confidentiality of all patients and organizational data in accordance with HIPAA.
  • Adhere to Connect Health + Wellness’s policies and procedures as well as federal and state regulations governing FQHC operations.
  • Participate in internal audits and implement feedback to improve accuracy and compliance.

Position Requirements

  • High school diploma or equivalent required; associate degree in health information management or similar field preferred.
  • Minimum of 2 years’ experience in medical billing and/or coding, preferably in an FQHC or community healthcare setting.
  • Working knowledge of CPT, ICD-10, HCPCS, modifiers, and FQHC-specific encounter billing practices.
  • Familiarity with eClinicalWorks (eCW) or comparable electronic health record and billing systems.
  • Ability to interpret Explanation of Benefits (EOBs) and electronic remittance advice (ERAs).
    Strong analytical, communication, and problem-solving skills.

Preferred Qualifications:

Certified Professional Coder (CPC), Certified Billing and Coding Specialist (CBCS), or Certified Professional Biller (CPB).

  • Bilingual (English/Spanish) preferred.
  • Knowledge of Medicare PPS and Medicaid Managed Care billing rules for FQHCs.
  • Experience with sliding fee scale programs and UDS reporting compliance.

Work Environment:
This position operates in a professional office environment with frequent use of standard office equipment.
Occasional travel between sites may be required.

Located in the foothills of the Blue Ridge Mountains in Southern Virginia, Martinsville-Henry County is about half way between Roanoke, VA and Greensboro, NC with plenty of activities.

Martinsville-Henry County is home to NASCAR’s historical Martinsville Speedway and has a culture of arts. The Smith River Trail system runs through MHC and includes several access points for fishing and kayaking.  A variety of outdoor activities can also be found at Fairystone State Park and Philpott Lake. The Smith River Sports Complex has walking trails, a canoe launch and a top-notch playground.


MHC is conveniently located at the junction of Highways 220 and 58 in southern Virginia.
