Connect Health + Wellness is seeking candidates for an experienced Licensed Practical Nurse (LPN) or an experienced CMA for a Float Nurse Position that will work at our Clinics in Bassett, Virginia, Ridgeway, Virginia or our clinic in the Patrick Springs community of Stuart, Virginia.


Connect Health + Wellness is committed to providing primary health, dental and integrated behavioral health services at our Federally Qualified Health Center sites and our Dental Clinics by promoting health, reducing health risk factors, and increasing access to medical and dental services, primarily for the uninsured and underserved in our service area. Additionally, Connect Health + Wellness offers chronic disease self-management, medication assistance, and care coordination services to our patients.


Our LPN’s and CMA’s provide clinical support in the delivery of quality care for the patient in a medical practice setting. Must have at least 1 year of experience in healthcare, preferably in a primary care or urgent care setting working with a Provider. Candidates should be familiar with the use of an electronic medical record (EMR), preferably eClinical Works (eCW), and knowledgeable about care coordination and health education.

Qualified candidates can provide professional nursing care according to current best practices, maintain confidentiality, promote person-centered care, communicate effectively and have good computer skills. Must be team oriented, a problem-solver and have excellent organizational and time management skills.


Job Duties

Job duties include, but may not be limited to the following:

Completes patient assessment to include height, weight, vital signs, chief complaint, current medications, etc.

Prepares patient for provider evaluation, including obtaining specimens, positioning for exams, listing medications, assisting during exams and diagnostic and therapeutic procedures.

Administers medications and injections as directed by the provider.

Maintains work area, exam, treatment rooms in a clean, safe manner; restock with supplies and instruments; sterilize reusable equipment as needed.

Documents patient information in the electronic medical recorded.

Provides patient teaching as indicated and directed by the provider including lab results, diet instructions, medication instructions, appropriate follow-up, and reinforcing provider instructions as needed.

Assist with referral authorizations for specialists and pre-authorizations for testing and ancillary services.

Assists with lab procedures.

Shift will vary based on need of the Clinic and Provider’s schedule.

Minimum Qualifications

Graduate of an accredited Practical Nursing program or CMA certification program.

Current licensure as an LPN or certification as a CMA in the State of Virginia required

Current AHA BLS-HCP Certification required

Minimum 1 years’ experience in healthcare, preferably in a medical office setting

Prior experience in data entry required

Connect Health + Wellness is seeking a compassionate and skilled Dentist to join our team in providing high-quality clinical dental care to a diverse patient population at our Federally Qualified Health Center (FQHC). The ideal candidate will have a commitment to serving underserved communities and be ready to work alongside dental students, staff, and other healthcare providers to ensure comprehensive, patient-centered care.


Key Responsibilities:

  • Clinical Care: Provide a wide range of dental services, including examinations,      diagnostics, and treatments.
  • Supervision: Serve as an Adjunct Faculty Member for Virginia Commonwealth University School of Dentistry, supervising dental students in clinical settings.
  • Patient Education: Educate patients on oral health, preventive care, and available      treatment options.
  • Collaboration: Work closely with the CEO, Medical Director, and interdisciplinary team to improve service delivery and patient care.
  • Policy Development: Assist in developing and revising dental department policies and protocols to ensure the best practices.
  • Community Outreach: Engage in community outreach to educate and provide technical assistance related to oral health.
  • Program Planning: Participate in planning initiatives for the Dental Clinic and      broader organizational goals.
  • Compliance & Quality Assurance: Ensure adherence to regulations and contribute to quality assurance efforts.
  • Consultation: Provide expert consultation on dental health issues to the CEO, Board of Directors, and medical teams.
  • Other Duties: Perform additional tasks as assigned by management.


Required Qualifications:

  • Education: Graduation from an accredited dental school.
  • Experience: Previous experience in a clinical dental setting is preferred.
  • Licensure: Current, unrestricted license to practice dentistry in Virginia.
  • Certifications: AHA BLS-HCP certification required; ACLS certification preferred.
  • Skills: Strong communication, interpersonal, and organizational skills. Ability to      work effectively with diverse populations and as part of a team.
  • Additional Qualifications: 
    • Bilingual in English and Spanish is preferred.
    • Knowledge of public health principles is a plus.
    • Proficiency with dental software and Electronic Medical Records (EMR).
    • Experience working with underserved communities is highly valued.

Our Family Nurse Practitioners provide comprehensive, evidence-based, primary and preventive medical care in a team-based environment to a diverse patient population with a wide range of health conditions and concerns. This position will be on day shift and may possibly work three 12-hour shifts per week or four 9-hour shifts per week based on our current needs.

ESSENTIAL FUNCTIONS OF THE POSITION

  1. Provides comprehensive, evidence-based, mental health therapy in a team-based environment to a diverse patient population with a wide range of health conditions and concerns. Performs mental health examinations within prescribed guidelines.
  2. Consults and works collaboratively with Provider, other behavioral health specialists and other medical professionals. Confers with specialists and/or consultants regarding the care of patients, as well as medical and diagnostic work performed, and coordinates the work of the consulting physicians with the work of the patient care team. Records findings and formulates a comprehensive patient care plan and prognosis, based on patient's condition and other related factors.
  3. Maintains required medical records and other documents in a complete, accurate and timely manner via the organization's Electronic Medical Record ('EMR').
  4. Participates in Quality Assurance initiatives and provides input on the development of policies and procedures that impact the health delivery service.
  5. Participates in training relevant to the education of professional health personnel as well as any training required by Connect Health+ Wellness.
  6. Provides exemplary customer service and represents the Coalition in a professional manner.
  7. Maintains the security and privacy of all protected health information.
  8. On average, achieves and maintains a minimum patient visitation rate of 97% as relates to established visitation goals.
  9. On average, achieves and maintains a patient duration of visit/cycle time of sixty-minutes or less.

Connect Health + Wellness - Dental is seeking candidates for a Registered Dental Hygienist.

POSITION SUMMARY

The Dental Hygienist performs various activities for a fast-paced Dental Clinic including assisting the dentist(s), and dental and dental hygiene students in the direct provision of primary care dental services to patients of the safety net dental clinic. The Dental Hygienist is responsible FOR Provide quality, comprehensive, and appropriate dental care and maintenance for the general health of patient's mouth and teeth.


ESSENTIAL FUNCTIONS OF THE POSITION

1. Assist in the oral health management of patients.

2. Greet patients in a welcoming manner, go over medical history, and take vitals and record patient's chief complaints in detail to go over with the dentist.

3. Performs independent procedures as delegated and directed by a dentist in accordance with state regulations and law.

4. Perform duties only appropriate to the education and experience of the dental hygienist and the practice of the supervising dentist.

5. Perform an initial examination of teeth and surrounding soft tissue including:

  • a. a head and neck exam,
  • b. charting of carious lesions,
  • c. probing depths,
  • d. signs of periodontal disease or possible cancers,
  • e. recessed & bleeding gums, and
  • f. any other abnormal conditions for further evaluation and diagnosis by the dentist.

6. Obtain oral hygiene assessment data and record patient-hygienist interactions in the patient's electronic dental record efficiently, accurately and in a timely manner following policies and procedures.

7. Take satisfactory and appropriate diagnostic radiographs that may include periapicals, bite winqs, and panorex as directed by the dentist.

8. Perform routine prophy, scaling and root planing of natural and restored teeth using hand instruments, rotary instruments and/or ultrasonic devices.

9. Polish and apply topical fluoride varnish to teeth as needed.

10. Utilize topical anesthetics either in gel or spray form as appropriate to minimize patient's sensitivity during scaling and root planing.

11. Remove sutures as directed by the dentist.

12. Apply pit/fissure sealants and silver diamine fluoride to patients exhibiting moderate to high-risk caries as directed by the dentist.

13. Provide oral health education and appropriate individual counseling for dental patients.

14. Educate patients in oral hygiene and dental care, including proper tooth brushing, flossing, nutrition and need for professional care.

15. Assist with or initiate emergency measures for sudden adverse developments during treatment of patients.

16. Provide service in a manner that is appropriate for the patient's age; demonstrate knowledge and skills necessary to meet the patient's physical, psychosocial, educational and safety needs.

17. Respect the dignity, confidentiality, and privacy of patients.

18. Ensure all patients enjoy a positive experience and are treated with care and compassion.

19. Prepare operatory for patient care by following prescribed procedures and protocols.

20. Ensure that dental scalers are sharpened on a routine basis for patient care.

21. Assist with the maintenance of dental equipment, assist with ordering and maintaining appropriate level of dental supplies.

22. Demonstrate knowledge and understanding of dentistry procedures, clinic infection control procedures, cleaning and sterilization of instruments, tray setup and materials.

23. Assist dentist as needed in a dental assistant capacity when the dental assistant is out.

24. Assist in reviewing clinic productivity, chart audits, and scheduling procedures.

25. Actively participate in community dental programs and outreach.

26. Assist with in-service and on-going training of clinic staff.

27. Build relationships and foster a high level of customer service.

28. Manage difficult clients and patients, demonstrating interpersonal savvy and influence skills.

29. Exercise professional judgment in the performance of services provided consistent with the organization’s policies and the standards of the dental profession.

30. Demonstrate a high degree of knowledge and competency in the practice of dentistry.

31. Perform assigned dental tasks using a high level of skill and ability.

32. Utilize and monitor effectiveness and efficiency of all equipment and supplies used in the delivery of services.

33. Communicate frequently with others in verbal and written communication.

EDUCATION AND TRAINING  

1. Graduate from accredited Dental Hygiene program

2. Successful completion the National Dental Hygiene Board Exam

3. Experience working in a primary dental setting, preferably with patients of all ages. 

4. Public health or community-based clinic experience preferred. 

5. Ability to attend to multiple tasks at the same time and to prioritize assignments and responsibilities to ensure compliance with established deadlines and protocols. 

6. Effective oral and written communication skills in English are required.  

7. Bilingual (English/Spanish) preferred but not required.


LICENSURE AND CREDENTIALS 

1. Holds a current, valid and unrestricted license to practice dental hygiene in the State of Virginia

2. BLS required.

Position Summary:  Provides comprehensive, quality, cost-effective mental health services to a diverse patient population. Participates in a Patient Centered Medical Home treatment team environment. Provides related services with the highest levels of quality and professionalism.

ESSENTIAL FUNCTIONS OF THE POSITION

  1. Provides comprehensive, evidence-based, mental health therapy in a team-based environment to a diverse patient population with a wide range of health conditions and concerns. Performs mental health examinations within prescribed guidelines.
  2. Consults and works collaboratively with Provider, other behavioral health specialists and other medical professionals. Confers with specialists and/or consultants regarding the care of patients, as well as medical and diagnostic work performed, and coordinates the work of the consulting physicians with the work of the patient care team. Records findings and formulates a comprehensive patient care plan and prognosis, based on patient’s condition and other related factors.
  3. Maintains required medical records and other documents in a complete, accurate and timely manner via the organization’s Electronic Medical Record (“EMR”).
  4. Participates in Quality Assurance initiatives and provides input on the development of policies and procedures that impact the health delivery service.
  5. Participates in training relevant to the education of professional health personnel as well as any training required by CHW.
  6. Provides exemplary customer service and represents CHW in a professional manner.
  7. On a rotating basis, acts as the after-hours on-call Provider.
  8. Maintains the security and privacy of all protected health information.
  9. On average, achieves and maintains a minimum patient visitation rate of 97% as relates to established visitation goals.
  10. On average, achieves and maintains a patient duration of visit/cycle time of sixty-minutes or less.
  11. Consistently participates in the peer review program, returning such reviews on a timely basis, and takes the initiative to hone skills and abilities to better meet PCMH goals and objectives.
  12. Consistently returns telephone calls, resolves and/or addresses EMR “bubbles” and other communications to patients and other staff and treatment team members and/or stakeholders within 48 hours.
  13. Participates in and/or supports organizational efforts to solicit unduplicated patients.
  14. Other duties as may be assigned from time-to-time by your supervisor that are consistent with Mission of the organization and intent of this position.


MINIMUM REQUIRED EDUCATION, EXPERIENCE, ABILITIES & CHARACTERISTICS 

  • Graduate of an approved and accredited school of social work or counseling.
  • Current, valid license for the State of Virginia.
  • Recognized by the Virginia Board of Social Work or the Virginia Board of Counseling as a LPC.
  • Current CPR and AED certifications.
  • Minimum of two years’ experience in a primary or behavioral health care setting.
  • Bilingual in English/Spanish preferred but not required.
  • Ability to relate well to people of various social-economic backgrounds.
  • Someone who is well organized, enjoys multi-tasking and thrives in a high energy, fast-paced environment.
  • Proficient with electronic medical records software programs.
  • Experience in eClinical Works electronic medical record system (preferred, not required)
  • Evidence of involvement and willingness to participate in community leadership activities.
  • A passion and concern for the target population served by the CHW.

Connect Health + Wellness, is seeking candidates for a full-time Bilingual Front Office Clerk at our Ridgeway, Virginia location.


Connect Health + Wellness is committed to providing primary health, dental and integrated behavioral health services at our Federally Qualified Health Center sites and our Dental Clinics by promoting health, reducing health risk factors, and increasing access to medical  and dental services, primarily for the uninsured and underserved in our service area. Additionally, Connect Health + Wellness offers chronic disease self-management, medication assistance, and care coordination services to patients in our service area.

Candidates must be willing to work Monday through Friday on a varied 8-hour per day schedule with hours ranging from 6:45am to 7:15pm.


Front Office Staff duties can include, but may not be limited to the following:


  • Welcoming patients and visitors by greeting them in a friendly manner, answering inquiries and directing them through the registration process
  • Screening patients and visitors (taking temperature and asking screening questions)
  • Checking in patients and preparing electronic chart for visit
  • Collecting and updating all patient demographic, eligibility, and insurance information
  • Entering all appropriate data into the Electronic Medical Records system
  • Explaining Connect Health + Wellness's services, as well as patient policies and procedures
  • Answering phone lines and maintains phone calls incoming to the office daily
  • Filing
  • Faxing and scanning documentation
  • Maintains fax in/out box and assigns appropriate paperwork to nurses, providers, and referral coordinator
  • Completing daily patient callbacks
  • Collecting all insurance information
  • Verifying patient demographics
  • Collecting and processing payments for services current and past, explaining patient charges as requested
  • Creating telephone encounters for medication refills, nurse messages, and chart inquiries
  • Assisting patients on their departure and scheduling follow up appointments
  • Entering medical, pharmacy, transportation, and ancillary service visits and enters data into the appropriate electronic medical records system
  • Obtaining record release signatures from patients to request outside medical office records. Follows up to ensure receipt of records for timely appointment scheduling
  • Works in conjunction with the Billing Coordinator to maintain knowledge about insurance policies and verifications continually
  • Answers questions regarding patient accounts, referral appointments, and upcoming office appointments
  • Explains charges to patient and directs any questions to the billing office as necessary.
  • Maintains accurate and up-to-date appointment system
  • Activates patient files
  • Provides support to medical staff
  • Maintains a clean waiting area
  • Discharging patients including verifying their information, and providing them with anything needed to leave such as discharge papers, prescription(s), or work/school excuses
  • Assists patients with scheduling appointments for Follow up, Rescheduled, Cancelled, and No-Show appointments
  • Checks encounter forms, medical records, and schedule to prevent double or missed appointments.
  • Maintains patient Sliding Fee Scale status with up-to-date POI and update the patient registration annually
  • Functions within the HIPAA laws to protect patient information protected
  • May be required to work from one of our other clinics based on need. 
  • Other duties as required

Located in the foothills of the Blue Ridge Mountains in Southern Virginia, Martinsville-Henry County is about half way between Roanoke, VA and Greensboro, NC with plenty of activities.
