Sliding Fee Discount Program

Connect Health + Wellness is committed to providing affordable healthcare and dental care for everyone in our community. Our Sliding Fee Discount Program helps eligible patients reduce the cost of medical and dental services based on household income and family size.

We welcome all patients, regardless of insurance status or ability to pay. No patient will be denied services due to an inability to pay.

Uninsured, underinsured, and high-deductible patients may qualify for discounted fees based on household income and family size.

To apply, complete a Sliding Fee Discount application at any Connect Health + Wellness location and provide proof of household income. Our staff will review your application and determine eligibility.

Once approved, your discount will remain active for 12 months. If your household income or family size changes, you may be asked to reapply sooner.

Accepted proof of income includes:

• Most recent federal tax return (IRS Form 1040)
• One full month of pay stubs
• Most recent W-2 form
• Unemployment benefit letter
• Social Security benefit letter
• Letter of Support
• Letter of Declaration
